Proof of Name: Is your current full name the same as the name listed on the identity document you selected in Step 2, Proof of Identity?

If your true full name is not listed on your identity document, you will need to provide a document showing your true full name. DPS and tag agents must be able to connect your current full name with your identity document.

Court-ordered or legal name change documents. Court-ordered divorce decree. A government-issued death certificate of spouse which includes a connection to your current full legal name. Legal adoption paperwork. Out-of-state REAL ID compliant driver's license or ID. Military ID card, Common Access card, Uniform Services ID & Privilege card including all branches of military personnel and dependents, not including Merchant Marines.

Proof of Social Security Number (SSN): Even if you've previously provided or proven your Social Security number, you will need to bring in documentation.

Proof of Address: To establish Oklahoma residency and residential address, you must present TWO printed documents that show your current name and current Oklahoma residential address.

IMPORTANT - You may use two documents of the same type, but they cannot be issued from the same agency, business or institution.

Rental/Lease Agreement that includes the original signature of the lessor or landlord. Oklahoma voter notification card or voter profile report or Selective Service card. Any document issued by an educational institution. Any document issued by an insurance company or agent. Paycheck, paystub, W-2 or 1099 tax form. A U.S. government-issued marriage certificate or license signed by a government official. A loan agreement, payment booklet/voucher, or load statement. Letter on company letterhead from an employer certifying that you live at a non-business address owned by the business or corporation. Any document DMV accepted as proof of legal presence which contains an address. Any document accepted as proof of identity, such as an Oklahoma Concealed Weapon Permit/Concealed Handgun License which contains an address.
